
and because of all the immigrations from pilgrims, maybe they desired for their new angel selves to be created in a more prosperous place. So naturally this led to a bunch of immigration from other unseen, crumbling kingdoms as well, such as Catarina and the Land of Reeds. In ds3 there’s the constant theme that the world is caving in on itself, especially around lothric in particular. maybe this is why Lothric developed the sort of angel-based religion. I guess this shows that the angels have some sort of sentience and that it’s either a form of metamorphosis that takes place after they die or just the afterlife for the pilgrims of Londor. so if the player kills her instead she’ll become hostile and attack them like all the other angels do. At some point before the final boss, the hag says she always dreamt of becoming an angel, and it’s revealed that when she dies (via the player killing her or old age) she eventually does become an angel.

I think it’s really interesting and sheds light on the life cycle of the pilgrims, as well as the sable church as a whole. It’s the stone humped hag you saw at the start of the dlc. (At last not in the way that the others were) This is why her Angel doesn't have a corresponding larva, she wasn't a victim of time displacement. Meanwhile, the old hag goes through her entire metamorphosis without our interference, unless we kill her of course. It's also why their deaths are more sad than violent, they know what's happening to them and are terrified. This is why you must kill the larvae to remove the angels, their present self can't exist if their past self is killed. First the pilgrim itself, then the larva as the pilgrim sheds its old form, and finally an angel. This means, effectively, that through the merging of past and present, we are able to see the evolution of the pilgrims in real time.

The Dreg Heap is in a land where time is fractured and folding in on itself, hence why the original Firelink Shrine, Earthen Peak, and Demon Prince, are still here despite being gone by the time of the Unkindled. So why are you seeing the larva if the Angel is already in the sky? Simple. If you look at the Larvae you kill to remove the Angels, you see that the larva itself is emerging from the back of a dead pilgrim. I know others have already answered this, but I'll give my two cents.
